All mental health information is treated as strictly confidential. We:

Special Protections
• Psychotherapy notes are not legally authorized to be released directly to patients
• SUD records shall not be used or disclosed in civil, criminal, administrative, or
legislative proceedings unless based on written consent, or a court order after notice
and an opportunity to be heard is provided
• Minor client records are subject to applicable laws
Data Security
• We implement safeguards to protect information, including:
• Password-protected access controls
• Physical security measures for paper records
PHI may be securely shared by patients, with Premier Vantage Mental Health, in-office, via the secure 727-910-5916 phone number, via the patient portal, or via the secure fax number 727-635-2238.
Email is not a secure method.
Retention and Destruction
Records are retained in accordance with applicable laws and professional standards. When no longer required, records are securely destroyed to protect confidentiality.
Breach Notification
If a breach of unsecured protected information occurs, affected individuals will be notified as required by law. We will take prompt steps to mitigate harm and prevent future incidents.
